Middle school athletics are about having fun, building community and connections, supporting healthy lifestyles, and encouraging school spirit. All students are invited to participate!
To participate in sports in the Î÷Ê©Ö±²¥APP School District, complete the following for each sport:
- Middle School Activity Participation Form
- $110 Sports Fee OR Fee Waiver Form
- Physical (good for 18 months from most current physical)
To register for sports:
- Print and fill out the following forms (links will go live when forms are updated for 24-25 school year):
- Paper copies of all forms are available in the main office. Submit forms to the front office.
To pay the $110 sports and activities fee online through Parent Connect:
- Log into Parent Connect.

The first box should be “ASD Quick Links.”

Things to Know:- All forms and payment must be submitted before students can participate.
- Athletes must have 10 CENTRAL practices to compete. (Outside activities DO NOT COUNT towards these 10 practices.)
- Sports practices usually begin at the start of the quarter.

- If the $110 activities fee creates a hardship for your family, complete this fee waiver form and submit it with your other activity forms.
