Clark School Based Health Center

  • heart and steth The Clark School Based Health Center is located at Clark Middle School and offers medical services Monday and Wednesday mornings during school, including:  

    • Immunizations
    • Sports Physicals
    • Treatment of minor illness and injury (for example, flu, colds, sore throats, sprains, cuts, burns, skin problems, earaches) 

    School Based Health Centers (SBHCs) help keep students healthy and in class so they can learn what they need to know to succeed in school and in life. Parents don't have to take time off work to transport students to another clinic or provider to meet these requirements. Students don't miss school for falling behind on immunizations, and don't miss out on participating in school sports for lack of a physical.

    The SBHC provides on-site medical services at school that include immunizations, physicals, and treatment of illnesses during the school day. Parents can register their students with the Î÷Ê©Ö±²¥APP SBHC by completing an enrollment packet (links below) and turning those forms into the school nurse.     The Î÷Ê©Ö±²¥APP School Based Health Centers (SBHCs) are a partnership between the Î÷Ê©Ö±²¥APP School District and Christian Health Associates. While we do bill for visits for physicals and treatment of minor illnesses and injuries, we serve families regardless of ability to pay and discount services based on income and family size. Using the SBHC is convenient for parents, who do not need to worry about taking time off work or arranging transportation to take their child to the doctor.

    WHY DOES CLARK HAVE A SCHOOL BASED HEALTH CENTER?
    The Clark community requested a health center during the planning process for the new school. A 240 sq ft health center space was constructed based on this request. Community health care providers and advocates worked with the Î÷Ê©Ö±²¥APP School District and Municipality of Î÷Ê©Ö±²¥APP to determine how to offer services in that space, and the Clark School Based Health Center pilot program was approved by the Î÷Ê©Ö±²¥APP Î÷Ê©Ö±²¥APP in fall 2009. Services are funded through grants and in-kind donations. 


    Parents do not need to make appointments for their student to receive services. Students will receive services as needed (for example, when they become sick during the school day) as long as the needed forms are on file.



    CAN HEALTH CENTER STAFF WRITE PRESCRIPTIONS FOR MEDICATIONS THAT MY CHILD NEEDS ON AN ONGOING BASIS?


    No. The school based health center will only write prescriptions for conditions treated in the school health center. If your child is being treated by another health care provider for a chronic health problem like asthma, you will still need to see that health care provider for a prescription to treat that condition.

     


    SERVICES WILL BE ASSESSED USING A SLIDING SCALE FEE.


    THE CLARK SCHOOL BASED HEALTH CENTER IS PROVIDED THROUGH A PARTNERSHIP BETWEEN THE ANCHORAGE SCHOOL DISTRICT AND CHRISTIAN HEALTH ASSOCIATES WITH GENEROUS SUPPORT FROM PROVIDENCE HEALTH AND SERVICES ALASKA, THE RASMUSON FOUNDATION AND COMMUNITY HEALTH CARE PROVIDERS.
